What is vaginal fatigue?

Dr. Brown defines vaginal fatigue as any disorder of function of the bladder and vagina. This can include unwanted bladder leakage, bladder urgency, vaginal dryness, vaginal looseness or relaxation, and Genitourinary Syndrome of Menopause.

Why does vaginal fatigue occur?

There are a number of predisposing factors that lead to Vaginal Fatigue. Collagen loss, childbirth or injury, and menopausal loss of estrogen, can all cause unwanted vaginal issues.

Vaginal Relaxation Syndrome (VRS)

It’s no secret that both having a baby and the natural aging process can change things below the belt. Defined as laxity of the vaginal wall, vaginal relaxation syndrome can cause a loss of friction and satisfaction during intercourse. Childbirth often contributes to VRS, but it can also result from natural aging and congenital connective tissue weakness.

Stress Urinary Incontinence (SUI)

You deserve to laugh, cough, or sneeze without worrying about leakage. Stress urinary incontinence is a condition that causes women to involuntarily leak urine. SUI can also affect women during physical activities, such as climbing stairs or running. There are several factors which can play a role in developing SUI, such as childbirth, obesity and genetics.

Vaginal Atrophy

If your sex life has been interrupted by itching or burning, it’s most likely the result of vaginal dryness. Estrogen levels decrease when a woman goes through menopause which can affect the bladder, vulva, and vagina. Many menopausal women also experience vaginal atrophy, a condition in which the walls of the vagina become dryer, thinner and more easily inflamed resulting in painful intercourse.

    Bladder Leakage or Stress Incontinence

    This involves unexpected loss of urine when the bladder is stressed with increased pressure. It is due to loss of collagen with ageing and childbirth injury which destroy the collagen support to the bladder neck. As the bladder neck descends with stress from laughing or coughing, leakage occurs. Increased pressure can be due to:

    Pelvic Organ Prolapse

    Damage during vaginal delivery is one of the primary reasons for prolapse. While prolapse can be seen in women without pregnancy or a vaginal delivery, it is very uncommon. The net effect of damage during childbirth is loss of the collagen supports in the organ prolapsing. Since the vagina is a ‘hollow’ space, organs tend to fall into the vagina. Prolapse can be graded in terms of how far down the vagina it has come when a patient strains.

    Grade 1

    1/3 of the way down the vagina and often has no symptoms.

    Grade 2

    2/3 of the way down the vagina and may be associated with a feeling of pelvic heaviness. This prolapse is often seen at routine pap exams.

    Grade 3

    Visible just outside the vagina and often detected by the patient.

    Grade 4

    Easily visible outside the vagina.
